Intensive Care Coordination (ICC / HFW)

High Fidelity Wraparound (HFW) 

EMS of Virginia provides Intensive Care Coordination (ICC) using the High Fidelity Wraparound model. This is an evidenced-based practice that assists youth and families who are facing challenges with the youth’s stability in the home environment. Primarily, this model is intended for youth who are at risk for residential or transitioning out of a residential placement. All of the ICC Facilitators and Supervisors have been specifically trained in the ICC HFW model.

The High Fidelity Wraparound (HFW) approach is a process of care management that holistically addresses the behavioral and social needs of a youth and family to develop self-efficacy. It provides the family with voice and ownership of their plan of care and service delivery. With the help and support of the facilitator, as well as youth and family supports, the youth and family develop their team. The team will consist of system partners and those important to the family (natural supports).

Our experienced, professional staff will provide an individualized service plan focused on the family and utilizing all available natural supports. This may include friends, other family members, neighbors, church associations, informal mentors, or anyone else that provides support to a youth and family “naturally” (not paid). EMS of Virginia coordinates care with natural supports and all systems already involved with the family to help the youth remain in their community and prevent future placements outside of the home. Such systems may include Department of Social Services, Community Services Board, School, Court Services, and/or providers of outpatient therapy, family therapy, intensive in-home counseling, mentoring, residential or group home placement, or a host of other services that might be in place. 

The family unit is central to goal development and we will work to ensure the family has access to any services they may need. Regularly coordinated meetings ensure our families understand the process, guide the progress, and that all systems work together to benefit the family and youth.

Intensive Care Coordination is currently funded through private pay and CSA funding through FAPT. The cost is $1300 per month for each youth.

For Community Partners

EMS of Virginia is offering Intensive Care Coordination through the High Fidelity Wraparound model in an effort to more broadly serve our community and collaborate with community partners. With changes to the System of Care funding in Virginia, we are aware that many agencies no longer provide ICC / HFW due to more limited funding. EMS of Virginia strives to fill this service gap and is welcoming referrals from CSA/FAPT or other avenues.
